Consulate Health Care of Jacksonville

Consulate Nursing Home in Jacksonville should not even be open for operation. My grandmother was there for a couple years and through the years we had several issues regarding staffing and cleanliness. When it really got horrible was when my Grandmother wasn't able to care for herself any longer. They are frequently understaffed and aren't safely able to care for the poor residents that reside there. My Grandmother was subject to neglect. She wasn't offered her basic needs such as water throughout the day or being comfortable and positioned in her chair. She would lay in bed all day, nobody would get her up unless our family went in and complained. Once staff got her up she would sit all day long. We would arrive to her crying and stating her bottom hurt. We reported her pain and were told “Its not even red” But when she was transferred to another facility they documented it as a Stage 1 pressure ulcer. She also sustained 3 injuries to her legs that they could not describe how it happened. They were not bumps or bruises they were open bleeding gashes. We were told “she hit her leg” My Grandmother is unable to move her arms due to contractures nor is she able to lift her legs. These injuries had to have been sustained while roughly transferring her. Its the only explanation for someone who can't physically lift their limbs. She had 3 separate injuries to her legs. The first 2 were open wound gashes to her shins. The third was on her leg behind her knee., indicating she was pulled up roughly in her Geri-chair and it got stuck. Staff frequently yell and complain in the halls. They complain they are short staffed frequently. While visiting we had one staff member tell us “I just don't know how much more I can take watching these aides rough the patients up” The staff are very intimidating especially if you report them for not doing their basic job. They circle family and glare and try to intimidate. Its a very volatile environment especially on the weekends
