Sage Rehab Hospital and Outpatient Services - Skilled Nursing Facility

Run! Can’t begin to explain the sheer incompetence of the entire staff and lack of communication during COVID. My father fell no less than 7 times during a brief stay. The staff kept him heavily sedated most of the time unless we had an appointment to visit. Not sure he even received any rehabilitation or very little. Finally had to get an advocate for the patient to get involved and finally a formal department of health and hospital complaint. Please please please do your research. No one should have to go through what my family endured.

I recovered from a major surgery at Sage and it was wonderful. The nurses and staff were all very caring. The food was pretty good. This was a much better experience for me than my previous stays in nursing care facilities.

Sage is a decent temporary, transitional facility for patients needing physical therapy before returning home. My mom had a debilitating stroke and required total care, meaning she could do nothing for herself. In less than a month at Sage, she had a stage 3 bedsore. She apparently was not turned often enough in bed and was left in a wheelchair for too long during the day. Her "diaper area" was also not kept clean. The doctors do almost nothing to oversee patient care, and the charge nurses are as neglectful as the majority of the care assistants. Sage is not the place for stroke victims.

This is the WORST rehab facility in the Baton Rouge area. We only chose them because we didn’t have much time to choose a facility. My Grandma is stubborn and becomes combative and refuses to eat or do anything sometimes and told us that the nurses was actually hitting her while other nurses held her down when she refused to do what they told her. If you have an ounce of love and compassion for your love ones don’t put them here if they can’t do much for themselves. The physicians have no bedside manners and the management is nonexistent.

This is the absolute worst place you can send a love one for rehab. The staff is overwhelmingly unprofessional. The cleanliness of the building is subpar and the patient bathrooms are horrible. There is no clear communication between the staff. The administrator never seems to know what's going on. I never write reviews but after the experience my Sister had here I would not want anyone to have to undergo this type of treatment. There are far better options for Rehab in Baton Rouge however we have to find out after moving my sister from Sage.
