Shenandoah Nursing and Rehab

My mother resided at Shenandoah's assisted living community from May 2013 until her death in January 2018. During that time, many of the staff became like extended family, & she generally received good care. About a month ago, my father needed skilled care, following a hospitalization. We were thrilled to find Shenandoah had an open bed, & confident he would receive quality care. To say we were greatly disappointed is putting it mildly. Dad has moderate dementia & very limited short-term memory. Using a call bell is not something he can retain. He also gets frequent urinary tract infections. Upon admission, I told staff that he would stay dry, & avoid infection, if they would go in every 4 hours, & assist him with toileting. Numerous times, we arrived to find his Depends so saturated it had spilled out onto his clothes. His skin became red & excoriated. Throughout his stay, we addressed the issue with numerous members of the nursing staff individually, the nurse manager of the unit in a care planning meeting, & the facility's nursing director. No significant improvement occurred. On the day he was to be discharged to an assisted living facility, I found him more confused, so weak he was slumped to the side in his wheelchair & could barely stand with the assistance of 3 people (myself & 2 CNAs), & he threw up within minutes of my arrival...symptoms that, for him, are hallmarks of infection. He also had a low-grade fever. When he further deteriorated, I asked staff to call 911 for transport to the emergency room. A short time later, the Director of Nursing arrived in the room & informed me she'd halted the 911 call, in order to first do her own assessment. She felt that the vomiting may have been due to something he ate, & that what for him constitutes a fever (99.1°F), wasn't a particular cause for concern. She also attempted to have him stand, despite his weakness & by then, his increased shortness of breath. I immediately put a stop to that, as my Dad also has very poor heart function, & called a private ambulance service for hospital transport. After I'd made arrangements, she later offered to call 911. A day later, Dad was diagnosed with C. Diff Colitis & put on antibiotics. I'm not sure what has happened to the quality of care here, in recent years, but I hope they quickly rectify it so that it again becomes safe, & no other residents experience what my dad endured.

They yell at the patients and the place smells like defecation. Our family member asked to go to the toilet and had to use his diaper instead. This is unacceptable.
