Southeast Rehabilitation and Skilled Care Center

I was horrified to hear that a family member had fallen out of wheel chair because one of the ‘nurses’ didn’t put the foot rest up. She was badly injured bleeding from her head nose and mouth. Beware of this facility. It’s a danger to put loved ones in!

This place is the worst of the worst. The staff is an absolute joke and shouldn't be allowed to take care of pet fish.

This place should be shut down. The staff is pathetic and uneducated. A family member walked to to visit my grandfather to find him in the middle of cardiac arrest, When the staff was told, instead of running and trying to save his life she replied with " I think I was just in there". When they did respond they didn't know how to put the bag together and my brother who is a medic had to do it. This all took place at 2:00pm and my grandmother recieved a phone call at 5pm saying that he was transported to the ER. This place should be shut down and the staff should not be allowed to work at a dog kennel.

On the rehab floor, patients are ignored for long periods of time. No one responds to the call button. Staff seems uncaring. Medication is regularly not given on time and I discovered that on a number of occasions doses of medication were not given at all. Patients are neglected and medical care is subpar. Communication with the patient and family is terrible. Rehab unit is understaffed and the CNAs are often rude and incompetent. It is often hard to find a nurse or CNA to discuss a problem with a family member. Despite being there for rehab (and despite charging Medicare for services) my family member went without OT or PT for days. The facility is dark, cramped, and old. The only common space is small, crowded, and dirty. In the kitchen area, the refrigerator is dirty and smells terrible. Much of my family members possessions (clothing, shoes) were lost and never found.

If I could give this place no stars I would!!!! Do not send your loved ones here!!!!!!!! My grandfather was in this facility for 3 days !!! We were told some horrific news that the tip of his finger was completely gone!! They couldn’t find the tip nor his clothing or bed sheets! We reported this to the state. This place needs to shut down!!!! DO NOT BRING YOUR LOVED ONES HERE!!!!!

I would not recommend this facility for many reasons.I moved my relative to a different facility because I was astounded the lack of cleanliness, lack of morale, delayed or unimplimented interventions and unpleasant staff, and most importantly very inadequate nursing assessment and delivery of care. To make it short it seemed they could of cared less. They made me second guess gut instinct until I sent my family member somewhere else where the care was wonderful from top to bottom and he thrived.

They are understaffed, the Nurses are overworked and underpaid, the majority of their CNA staff is underpaid and underskilled. There's an overall shady and slimy attitude presented by the administration. They prey on the diminished mental capacity of the elderly in their care and try to manipulate them and their family members by purposely telling the patient one thing, and the family another. They lie, they double-talk, and they withhold information from patients and family. Important information like Insurance practices and basic patient's rights. The worst is the "Administrator" Blaming patients for their CNA's ineptitudes, negative and disgusting atittude, and talking down to patients as if they were toddlers. She should be fired and replaced.
