THE PICTURES LIE DON’t BE FOOLED This place is not well suited if you have special.dietary needs such diabetes or renal failure their set menu has no options for substitution it’s just what they bring you and the so called ala carte is the same choices of a hotdog, peanut butter and jelly sandwich , quesadilla (cheese only) grilled cheese with bacon and tomato, turkey and ham deli style, fruit and cottage cheese or chef salad with dressing definitely not foods one would eat when recovering and trying to heal the main issue is that some of the staff are under the impression that things are not their responsibility there is little ownership of the mandate to care for patients in addition the transportation they use are given incorrect times by Wellspring causing me to miss treatments also I was not given my insulin when needed even after advising the nurse that my blood sugar was high spending hours in a wheelchair, I was told by staff that getting dishes back to the kitchen was the priority not getting out of my chair after hours then to be told I couldn’t even get a Tylenol because the dr hasn’t approved anything the ratings and pictures on various sites and their website I saw lied , DO NOT BRING/SEND anyone you care about here.CASE MANAGER Anita is HORRIBLE at her job her priority seems to be getting you out the door , she doesn’t return phone calls , doesn’t follow up on things such as securing medical equipment needed for your discharge DO NOT BELIEVE A WORD SHE SAYS !!!! You will be doing all the work yourself or it will not get done THE ONLY REASON I SAW MY SURGEON WAS I BOOKED THE APPOINTMENT AFTER THREE WEEKS OF BEING HERE OTHERWISE THEY WERE JUST FINE DISCHARGING ME WITHOUT A DOCTORS SING OFF CNAs are under staffed and do all the work the NURSES here can’t be bothered unless they are pushing unneeded medications to pad your bill , they ignore patient call lights literally sitting on their duff while patients are yelling for assistance The Physical Therapy were GREAAT it’s sad the majority of the staff doesn’t display the same amount of positivity energy and care Be advised you will not see a Doctor here only Nurse Practitioners . I’m being discharged without my needed equipment no notification to my doctor for follow up just a pathetic see ya later good luck try not to end up in the hospital.

Absolutely loved this facility and staff! They treated my Aunt very well, and she raved about the meals they would serve for the patients! Thank you, Wellsprings Staff for all you did for my aunt!

My mom just discharged from this facility and she was so very happy! During this pandemic I wasn't able to be with her every day but the facility was communicative, helpful and caring to her! The staff are lovely, she said the therapy was top notch and the food was well balanced, tasty and she even had choices! Thank you to Wellsprings of Phoenix for a job well done! My mom is a tough one too! I will highly recommend you for anyone who might need your services.

My mom contracted cornavirus strand b here during her stay in feb 2020. She is now on a ventilator in icu and going to die. If you love your loved one find a different place

This facility is by far one of the best in the area! I read the most recent review and others and this family must be at the wrong facility. My mom is a patient here and was a patient at Wellsprings last year too and we receive 5 star treatment and I see all other patients treated the same. I think this "advocate" has a vendetta against Wellsprings. Do not trust everything you read, some people are "psychotic"! I give Wellsprings 5 stars each and every time my mom has been here!

I cannot believe that Medicare allows such treatment for Seniors. Wellsprings, has proven to provide the worst nursing care I have ever encountered. As a nurse, myself, the lack of professionalism & good nursing practice is shameful! Anyone providing a high review of this facility must not visit or speak to their relatives housed there. Patients are all treated like they all have dementia so staff don't worry about meeting patient requests or needs. Ask the Dir. or Assist. Dir.of Nursing what kind of response they give to patient complaints...it's called "silence". No compassion lives at Wellsprings!

My 93 year old mother was sent to the Phoenix facility because of a fall. The facility is well appointed and the lobby is welcoming but my family and I were not pleased with the care. She ended back in the hospital because they did not treat her uti and her wound properly. On numerous occasions we tried to speak to the physician in her care but only once did I see him and NEVER did he respond to my daughter's requests who happens to be a physician in the community and her POA. That is not only disrespectful but a disgrace to treat another physician that way!The doctor never ordered a psychiatric evaluation for dementia as he said he would do, so every time she refused treatment or care they listened to her! They even told her that she can decide where she wants to go! That was a social worker talking to a 93 year old woman who can't tell you what day, month or year it is! The nurses and assistants were caring but their hands are tied. Then when she was ready to be released to a care facility, at the last minute they said it wasn't an appropriate placement, so I had to scramble to find a new place because they charge $250 a day for each day after the discharge date, even though they recommended the facility! Because of the lack of care her health has declined because of a severe wound. I find the whole operation of this facility very disorganized and unless you keep on top of things, your family member might be discharged with no where to go and in worse shape then when she was admitted.

Chris Cunningham please read. Dawn who is the Administrator is a bully & mentally abusive to her employees with the help of Amy unless you are one of her favorites. If one of her favorites messes up she takes it out on the other employees. She leaves 2-3 times a month claiming she has to attend board meetings. She also opens everyone’s mail including the residents & checks everyone’s email & voicemails. She does not have what it takes to be an administrator & her favorites mess up all the time but the ones who pay for it is the hard working employees that aren’t taking advantage of the company.Amy is HR/Admission & she is one of Dawns favorites. She takes advantage of the company by never submitting PTO when she takes time off. As well as only going marketing when the owner is in the building. Besides that she’s usually in the building online shopping. Also $280.00 of the residents money went missing the day before she took a weeks vacation. Also residents accounts were fraudulently charged after she admitted them & she is the only one who has their information. Coincidence...I am thinking it is not.Erik is also another favorite who does the social work. Most of our bad reviews are because of him & we are threaten that we will be turned in Medicare because of them & him. He constantly takes from the company by only working 4 hours a day but gets paid for 8. He also pushes his work onto other employees & if they don’t do it they get in trouble. He also in the last 4 months has not submitted PTO even though he has taken at least one-two day off a week. These are just a few examples of how these three take advantage of the company & also how bad the other employees are treated & it is not fair. Dawn also tells everyone to make sure they check in & out on time but the day after she says this these 2 show up to work at 11am & Erik calls off.They need to be held accountable & stop taking advantage of the company & also the other employees.
