Home health care, which includes both help with day-to-day homemaking tasks and light medical services, costs $5,339 in Oregon, compared to $4,385 per month for the national average.
Home care services in Oregon cost more than the national average, but there are neighboring states with higher fees. The cost of home care and home health care services in Washington is more than $500 per month higher than in Oregon. Utah's fees for home health care are similar to Oregon's, but home care fees are $113 per month higher. Nevada and Idaho are the most affordable of the neighboring states, with monthly home care fees closer to the national average.

In addition to home care services, Oregon seniors can also opt for care in a group setting at adult day care centers or receive greater levels of support at an assisted living facility or nursing home. Of these options, adult day care is the most affordable, at less than half the cost of in-home care services. Assisted living services, which include room and board as well as help with day-to-day tasks, cost an average of $4,499 per month. Nursing home care is the most expensive, averaging $9,551 per month.
Home care costs vary across Oregon. Grants Pass, in the southern part of the state, has the lowest fees, with home care and home health care both costing $4,671 per month. Fees in Eugene are also lower than average, at $5,005 for home care, and $5,243 for home health care, respectively. Home care costs in Albany are close to the national average. The most expensive city for home care services is Bend, where fees are $5,625 per month. Fees in Portland are also far higher than average, with seniors being charged $5,590 for home care and $5,720 for home health care. Other cities in the northern half of the state, such as Salem and Corvallis, also have prices higher than the state average.

The K Plan is a Medicaid waiver that offers home and community support for eligible individuals as a part of the state Medicaid plan. Seniors who require nursing home levels of care can receive support with the activities of daily living, including bathing, grooming, housework, meal preparation and other day-to-day tasks. Community transportation, home modifications and assistive devices are also available under the plan.

This program is open to Oregon Medicaid recipients and provides eligible individuals with monthly cash payments that can be used for self-directed care. The Independent Choices Program is available statewide, allowing individuals to hire a caregiver, including spouses, adult children or other family members. To be eligible, seniors must be in receipt of Medicaid, require a nursing home level of care and meet income and asset requirements set relative to the Federal Benefit Rate.

Oregon Project Independence is a non-Medicaid program offered across the state. It's open to seniors who have a qualifying diagnosis, such as Alzheimer's, Parkinson's, or Multiple Sclerosis. Seniors can receive assistance with the activities of daily living and home care services. Anyone hired as a caregiver must be qualified and have a provider number.

Home care providers in Oregon are not formally regulated. However, anyone wishing to receive home care funded through Medicaid must hire an approved caregiver who has completed a training course and passed a criminal background check. Home health care providers are regulated by the Oregon Health Authority.
Scope of Care | Home care aides offer in-home assistance with the activities of daily living, including light housework and assistance with grooming, eating and toileting. Home care aids don't provide medical services but may assist with self-medication. Home health care providers offer similar services but may also provide medical care within the scope of their training. |
Care Plan Requirements | Home health agencies are required to perform an initial visit and draw up a plan of care for patients. The plan of care must be agreed upon with the patient's physician. Any modifications to the plan must be made with the physician's agreement, and the plan must be revisited regularly to ensure it's still suitable for the patient. |
Medication Management Requirements | Home care providers may assist with self-medication but are not permitted to administer medication or perform any medical procedure. Home health aides may administer medication if that medication is prescribed by a physician and is a part of the patient's plan of care. Any use of medication should be recorded in the patient's clinical record, and concerns about side effects or other issues must be reported to the physician. |
Staff Screening Requirements | Home care agencies that are a part of the Medicaid program are required to have all staff complete criminal background checks. Home health staff are also required to pass criminal background checks. |
Staff Training Requirements | Home care staff hired through Medicaid are required to complete an orientation course that covers health and safety, emergency procedures, best practices, safeguarding and other issues. Home health providers are required to complete extensive training relating to their clinical areas, such as occupational health or nursing, and must only provide services within their area of expertise. |
